Why Girls Tear ACLs More in Soccer

Most soccer parents have heard that girls tear ACLs more often than boys.

Fewer have heard the part that matters: how much of that gap is trainable.

Start with the honest framing

An ACL tear is not a training failure, and nothing here prevents every injury. Contact happens. Bad luck happens.

But the risk is not fixed, and the evidence on shifting it is unusually clear for this field.

What the evidence shows

Structured neuromuscular warm-up programmes — the FIFA 11+ family is the most studied — have produced injury risk reductions in the range of 32% to 56% in randomised trials across amateur soccer.

In preadolescent female players specifically, a randomised controlled trial found the programme improved knee and hip mechanics during single-leg landing, in the directions associated with less ACL stress. Other work found improvements in knee valgus — the knee falling inward — in female players compared with controls.

Knee valgus on landing and cutting is the mechanism people are describing when they say girls are at higher risk. It is also the thing that responds to training.

Why so few teams get the benefit

This is the uncomfortable part.

The reviews consistently report moderate adherence and limited uptake. The programme works; it just does not get done — or it gets done in August and quietly disappears by October.

It is free. It needs no equipment. It takes about fifteen minutes. And it is the single best-evidenced thing available to a youth soccer team.

What it means for your player

Two questions are worth asking her club.

Is there a structured neuromuscular warm-up, or a jog and a stretch? They are not the same thing.

And is it still being run in March, or did it stop when the season got busy?

If the answer is no, that gap is fillable outside club — landing mechanics, single-leg strength, and controlled deceleration are the components that carry the effect.

The cut is the moment. Train how she plants and turns, and you have trained the thing the injury actually happens in.

Questions parents ask

Is this something she will grow out of?

The risk pattern appears around adolescence and does not resolve on its own. What changes it is training — specifically neuromuscular work that improves how she lands and how she turns.

Her club already warms up. Isn't that the same thing?

Usually not. A jog and some static stretching is not a neuromuscular programme. The structured programmes that produced these results have specific strength, balance and landing components, done consistently.

Should I ask her club to run this?

Yes, and it is a reasonable ask — the programmes are free and published. The known obstacle is not cost or equipment, it is adherence: clubs start well and drift by mid-season.
